i could not answer #1 & 2

i moved from the exurbs to a small community - can approximately 350 people in 2 communities on our island be much smaller?

on my end maybe there are 100 people

we may be remote the nearest supermarket etc is 20 miles away

but we are on a commercial harbor
my nearest neighbor has a bait & tackle store with a little bar & grille about a quarter of a mile away - i can see it from my upstairs window

I would like to live in a small country town but what I would miss most would be good medical services.
That's what stops me from moving o the country

I've lived in communities varying in size from a small town (Socorro, New Mexico) to a big city (Phoenix, Arizona). I think I prefer a happy medium; where there are a lot of stores and modern conveniences but not so big that it takes a century to drive across town. I remember those long, long childhood drives to go visit friends in Scottsdale. My city (Albuquerque, New Mexico) is about the right size for me or a bit bigger. It was also my experience that having to find a job in a small town sucks, while people whose families are established in the town are shoehorned into jobs by nepotism. That's why I wouldn't live in a small town again. I have also heard that drama and gossip tends to be rampant in smaller towns.

I'd miss decent medical services (I can't say "good"), finally working taxi company (I've needed it at times), people who are close enough to come help me rather than being so far away that they can't come to my house or where I am, a library, random social gatherings, live theatre (both helping with plays and viewing them), Al-Anon meetings, easily attendable church services, easy access to a vet . . . I'm sure there's more that I'm forgetting.
